Plan A - There is a $10,000 Death Benefit for participants eligible for Plan A coverage at the time of their death. See Page 77 through 80 of the Summary Plan Description for the Active Health Plan A Booklet.
Plan B - There is a $5,000 Death Benefit for Participants eligible for a Plan B coverage at the time of their death. See page 86 through 89 of the Summary Plan B Description Booklet.
If you would like to designate a beneficiary, please complete and return the Participant Data Form to the Administrative Office.
