Important information about the Plan
|
You must have all dental services performed by a participating United Concordia dentits. If you go to a dentist other than a United Concordia network dentist, you and/or your denpendents will have no coverage for those services rendered, except for certain dental emergencies.
There are no claim forms to fill out, and the Trust Fund pays United Concordia directly for your dental care needs.
For any service performed by a specialist (Periodontist, Endodontist, Pedodontist & Oral Surgeon), you pay 75% of the specialist's contracted fee. For more information, contact United Concordia Member Services.
